We are seeking a skilled Registered Nurse to join our team at Baptist HomeCare. • Key Responsibilities:
• Provide and coordinate patient care in accordance with physician orders and agency policies
• Conduct initial and comprehensive nursing evaluations to determine primary focus of care
• Develop and implement individualized plans of care to meet patients' unique needs
• Collaborate with other healthcare professionals to achieve continuity of care and ensure seamless transitionsRequirements:
  • Current RN licensure in state of practice
  • Current CPR certification
